City are currently 11 points clear at the top of the table having won a record 15 consecutive matches.
Guardiola clinched the honours for September and October thus becoming the first manager in history to win it thrice in a row.
Guardiola topped a six-man shortlist including Antonio Conte, Sean Dyche, Jurgen Klopp, Jose Mourinho and Arsene Wenger.
Salah, meanwhile, was in devastating form for Liverpool in November.
The Egyptian struck eight times in four games, including three braces.
The winner was selected by a combination of votes from a panel of experts and the public.
